Nursing Home Admissions
We understand that preparing a loved on for admission to a Long-term Care facility can be extremely overwhelming for both the family and the resident. Being prepared with the right kind of information will reduce admission stress so you are able to concentrate on the healing process. A physician must certify the need for nursing care for insurance purposes. Our facility is open to all people, you need not have and Odd Fellow affiliation to receive care.
 
 
 
 
Personal Information
  • Resident's current address and phone number
  • Address and phone number for an family members that may assist during resident's stay.
  • Copies of healthcare directives 
             - Durable Power of Attorney for healthcare and/or Finances
             - Healthcare Proxy Designation
             - Living Will
 
Below are several downloadable documents that can be filled out and brought in on the day of admission.
 
 
 
Personal Items
Our goal is for your loved ones to enjoy the comforts of home. For this reason we suggest bringing the follow items:
  • Toiletries, i.e., hair brush/comb, toothbrush and paste, deodorant, baby powder, lotion, shampoo/conditioner, body soap
  • Clothing, such as, shirts, pants, undergarments, socks/nylons, belts, pajamas, non-skid slippers, comfortable walking shoes
  • We suggest marking clothing items with resident's name.
 
 
Financial Information
It is our goal for you to receive all of the healthcare benefits that you are entitled to; therefore it is very important for you to have your financial information in order. To setup the proper billing and continued long-term care benefits, such as Medicaid or Medicare, please provide the facility with following information at the time of admission.
  • Copy of Resident's Social Security Card
  • Copy of Resident's Medicare, Medicaid, or Insurance card
  • Financial Power of Attorney
  • List of Health Insurance Policies, including their coverage limits
  • Copies of pending Medicaid Title XIX application
  • Listing of income sources and/or assets (i.e., retirement, investments, banks, real estate)
